County Clinic offers Annual Wellness Visits
This visit is an opportunity for
your health care provider to work with you for prevention and early detection
of illness.
You are eligible to receive this benefit at no cost to you once a
year.
What
is a Wellness Visit?
- A review of your medical and family history
- A complete list of your current providers and
prescriptions
- Height, weight, blood pressure, and other
routine measurements
- Screening for cognitive impairment
- Personalized health advice
- A list of risk factors and treatment options
for you
- A screening schedule (like a checklist) for
appropriate preventative services
Your
health care team will provide you with information and referrals to maintain
your health as well as build your personal prevention plan that will guide your
care for the next five to ten years.
The benefit of having the Annual Wellness
Visit every year is that we can monitor your progress and identify early
changes in your health to prevent disease.
Please note that this is
different from a yearly physical checkup and no physical exam, labs, or other
testing will be done.
Who
pays for this this Wellness Visit?
Medicare
covers the full amount of this service. You will not be responsible for a
co-pay.
However, the doctor or care manager may recommend follow up doctor
visits that may require a copayment.
How
do I schedule my Annual Wellness Visit?
When
you call our office, please tell the staff that you would like to schedule your
Annual Wellness Visit.

A few days before your appointment, a member of
our healthcare staff will call you to complete a medical history and screening
questionnaires. If possible, please be prepared with the following information:
o
A
list of your current medical providers and suppliers (i.e. specialists,
pharmacy, medical equipment suppliers, etc.)
o
Current
medical problems
o
Past
medical history, surgical history, immunization history, family history, social
history
o
Current
medications and allergies

9 a.m.- 4 p.m. Wednesdays
“Great Plains of Cheyenne County, Inc.
